What Is the GUIDE Program?

GUIDE stands for Guiding an Improved Dementia Experience, a care model developed by the Centers for Medicare & Medicaid Services (CMS) to enhance the quality of life for individuals living with dementia. The GUIDE Program supports patients and their caregivers by offering:

• Dedicated Dementia Care Navigators

• Personalized care plans

• 24/7 access to support

• In-home and virtual visits

• Caregiver training and education

• Respite care services up to $2,500 per year

At MPS, we’ve integrated the GUIDE Program into our in-home care model to ensure that patients receive coordinated, compassionate dementia care that aligns with their unique needs and goals.
