The GUIDE Program is a new Medicare benefit that brings dementia care right to your door. It’s only available through approved healthcare providers, and Mobile Physician Services is proud to offer it throughout Florida and Ohio.


What Is the GUIDE Program?

GUIDE stands for Guiding an Improved Dementia Experience. It’s an 8-year nationwide care model from Medicare (CMS) designed to support people living with dementia and the people who care for them. The goal is to help patients stay safely at home longer, reduce hospital visits, and improve the overall quality of life for both the patient and their caregiver.

This program offers complete dementia care coordination, which includes everything from clinical support to caregiver education and even short-term relief options known as respite care.

Mobile Physician Services is one of the few practices in Florida selected to participate in the GUIDE program. That means we’re authorized by Medicare to bring these extra services to our patients’ homes.


Who Can Join the GUIDE Program?

To qualify for GUIDE services, a patient must:

  • Be enrolled in Traditional Medicare (Parts A and B)
  • Not be enrolled in Medicare Advantage, hospice, or PACE
  • Not on Hospice
  • Not residing in a traditional nursing home

What You Get Through the GUIDE Program

Patients and caregivers who are approved for GUIDE receive all of the following at no extra cost:

  • In-home medical visits from a dementia-trained care team
  • 24/7 phone support for urgent needs and questions
  • ✅ A full Comprehensive Dementia Assessment done in your home
  • ✅ A custom care plan tailored to your health, memory status, and goals
  • ✅ Help managing medications and symptoms
  • Caregiver training to help family members feel more confident and prepared
  • Coordination with community services like meals, transportation, and adult day programs
  • Respite services, which means short-term relief care so your caregiver can rest
  • Yearly check-ins and updates to your care plan

Medicare pays for these services as part of a bundled care payment model. That means patients don’t get billed separately for each visit or service.


How the Program Works

Here’s how we make it simple for you:

  1. You contact us to ask about the GUIDE Program
  2. We schedule a home visit to evaluate your situation
  3. You give consent to join the program if you qualify
  4. We submit your information to Medicare
  5. Once approved, we begin monthly care and support right in your home

The program is completely voluntary. You keep all your other doctors and services — GUIDE simply adds an extra layer of specialized help.


Understanding GUIDE Tiers

Every patient in GUIDE is placed into a tier based on their dementia stage and caregiver needs. Medicare uses these tiers to determine the kind of services and support the patient will receive. Tiers range from lower needs to higher needs and help ensure the care you get matches your situation.

For example:

  • A patient with mild dementia and low caregiver stress might be placed in a “Low Complexity Dyad Tier.”
  • A patient with more advanced dementia and a very stressed caregiver might be in a “High Complexity Dyad Tier.”

This tiering system ensures resources go where they’re needed most.
